For years, parts of the recovery world and parts of the mental health world have often talked past each other. That has improved. Most serious clinicians today understand what many people in recovery have known for a long time: substance addiction and mental health struggles often travel together, and both deserve compassionate, evidence-informed care. I believe that. I also believe many people in long-term 12-Step recovery have been helped greatly by therapy, psychiatric care, and, when appropriate, medication.

This is not an argument for or against psychedelic treatment. It is a reflection on risk, fit, and discernment. For some people, these interventions may hold promise where other treatments have failed them. But for those in long-term 12-Step recovery from substance addiction, the question is not just whether a treatment may relieve symptoms. It is whether it could destabilize the very patterns, boundaries, and spiritual practices that have helped them sustain their sobriety.
